The Social Care Network Program helps people access support through NYS's 1115 Waiver. Services include food, housing, cooking supplies and other essential needs. Eligibility and availability depend on your situation and location.

This support is designed for individuals and families enrolled in Medicaid who are navigating complex systems in order to manage ongoing health needs.
Care management can help with:
  • Coordinating appointments, providers, and next steps
  • Understanding letters, paperwork, and benefits
  • Connecting to medical care and community resources
  • Navigating housing, food access, transportation, and daily-life needs
  • Staying organized and following through over time

What to Expect

You’ll be matched with a care manager who works with you over time. Together, you’ll talk through your situation, set priorities, and take next steps at a pace that works for you.
Your Care Manager stays connected as things change, helping you adjust and move forward. Support continues for as long as you choose to stay enrolled in the program.
Our team understands the New York healthcare and community support landscape and works closely with hospitals, mental health providers, specialists, home care agencies, and community organizations across the region. We help people understand their options, navigate insurance and eligibility considerations, and connect to care services that fit their needs and situation.

Social Care Network (SCN) Support for Health-Related Social Needs
We provide services through the New York Social Care Network (SCN) program. This helps address everyday needs that affect health and well-being beyond medical care.
This includes the following supports at no cost to you:
Transportation
Nutritious meals & pantry stocking
Housing support & housing stability
Employment & education 
Financial & other essential needs

Start by completing a short self-screening i to learn what services may be available to you; or reach out to us and we’ll walk you through the screening.
Depending on your situation and location, these services may be provided directly through Lighthouse Care or through trusted partner organizations.
How This Works Together
Care management keeps everything connected.
SCN provides real, tangible, immediate support.
Your care manager helps you stay organized, understand your options, and coordinate across everything you’re managing.
Through the Social Care Network (SCN), Lighthouse Care provides food, housing, transportation, and other supports as part of the SCN program.
Either way, you’re not navigating these systems on your own.
